Graduate Course Selection Approval Form
1. Before selecting courses each semester, approval from your advisor is required for the following courses:

【Master’s Program】
1. Required courses (choose two from the following): Solid State Physics (I), Quantum Mechanics (II), Electrodynamics (II), Statistical Mechanics.
2. Elective courses from other departments (up to 3 credits).
【Doctoral Program】
Elective courses from other departments (up to 11 credits).
2. If there are changes in courses after adding or dropping, a new form must be completed and re-approved by the advisor.
3. Keep this form for your records after adding or dropping courses. During the graduation credit review, the department will assume that the courses you selected have been approved by your advisor. If it is found that courses were selected without the advisor’s approval and it affects your graduation, you will be responsible for the consequences.
4. Please complete the procedures according to the above regulations.
5. If you have not yet determined an advisor, the department chair will sign this form on your behalf. -
[Graduate Course Credit Certification Form] New graduate students who did not graduate from the NCKU Department of Physics and are applying for **credit exemption** must include this form as one of the necessary attachments.
Graduate students from other institutions who wish to exempt courses in the NCKU Department of Physics must first download the [Graduate Course Credit Certification Form]. After completing it with their previous institution, they should submit the form through the NCKU university system for credit exemption. The form must be uploaded as one of the attachments in the NCKU exemption system; otherwise, the exemption will not be granted.

1. This course selection form does NOT apply to general education courses (A9), physical education courses (A2), and military training courses (A3).
2. Please fill in the information carefully, especially the "Course Selection Number" (5 digits in total, such as C2-000, L2-000). After completing the form, send it to the course selection undertaker of the target department (institution). 【The Department of Physics can only handle courses selection number starting with C2 and L2】.
3. For【General Physics Experiment】, the 【Signature of the Manager of the General Physics Laboratory on the 2nd Floor of the Physics and Chemistry Building of the Department of Physics 】must be obtained (not the lecturer’s). As to professional experimental courses of Department of Physics, 【Please ask the teaching assistant of the course】. For other courses, the signature of the lecturer must be obtained in advance. If the number of adding-in request exceeds the capacity of the classroom, the add-in order will be based on the application order received by the department office.
4. Students who wish to withdraw, please do the withdraw process online while the online course selection system is open.
5. Please complete the course selection process within the course selection period announced by NCKU. After the deadline, the system will be shut down and the department office can’t do any adjustment!
6. Please complete your course selection confirmation online before the deadline announced by NCKU. Those who did not check their course selection status online should not have any objection in the future.
